Important Notice to Applicant

In connection with your application to provide tutoring, teaching, or educational services through Kids on the Yard (operated by Limitless Virtue LLC), we wish to inform you that a consumer report and/or investigative consumer report may be obtained for employment purposes as defined under the Fair Credit Reporting Act, 15 U.S.C. Section 1681 et seq. ("FCRA").

What This Means

A "consumer report" is a communication of information by a Consumer Reporting Agency (CRA) bearing on your creditworthiness, credit standing, credit capacity, character, general reputation, personal characteristics, or mode of living. This report may include, but is not limited to:

• Criminal history records (felony and misdemeanor convictions)
• Sex offender registry searches
• Social Security Number trace and verification
• County criminal court records (past 7 years)
• Nationwide criminal database searches
• Domestic watch list searches
• Identity and address verification

Your Rights

Under the FCRA, before we take any adverse action based in whole or in part on information in a consumer report, you will be provided:

1. A copy of the consumer report; and
2. A description in writing of your rights under the FCRA (A Summary of Your Rights Under the Fair Credit Reporting Act).

You have the right under Section 606(b)(2)(B) of the FCRA to request disclosure of the nature and scope of any investigative consumer report and such disclosure will be made to you upon request within five (5) days of the date on which the request was received or the report was first requested, whichever is later.

Acknowledgment

By proceeding with this application, you acknowledge that you have read and understand this disclosure. A separate authorization form will be presented for your signature.

This disclosure is provided as a standalone document in compliance with 15 U.S.C. Section 1681b(b)(2)(A), which requires that the disclosure be made in a document that consists solely of the disclosure.
